1. Claim Form completely filled up.
2. Attested copy of membership certificate.
3. Attested copy of Firm Registration Certificate/PAN Certificate.
4. Emergency ticket/prescription of hospital, where he/she was carried for initial treatment.
5. Death certificate issued by hospital.
6. Police report in original.
7. Attested copy of 'Las Janch Prakiti Muchulka'.
8. Attested copy of Postmortem Report.
9. Attested copy of Death Certificate issued by local authority.
10. Attested copy of Succession Certificate (Nata Pramanit).
11. Attested copies of citizenship certificate of deceased person and legal heir.
12. Attested copy of driving licence and blue book.
13. Other documents, if any.
